Liability and Consent

I/we, the Parents/Guardians of the Child(ren)/Youth Mentioned previously, authorize one of Spring Garden Church's Ministry Personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures if needed within an emergency where I/we are not present

Consent *

I/we, undertake and agree to indemnify and hold harmless Program Personnel, Spring Garden Church, and its leaders from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Spring Garden Church, as well as of any medical treatment appropriately authorized by the supervising individuals representing Spring Garden Church. This consent and authorization is effective only when participating in or traveling to events sponsored by Spring Garden Church.

Communication with Youth

A policy states that social media networks and text messaging are great entry points for building relationships
with youth. Every effort will be made to increase interaction off cyberspace. Communication with youth (grade 6-12) or 12 years of age or older via social media, telephone and texting is permitted under the following conditions:
a. Ministry Personnel are to secure written permission from parents to communicate with youth. Permission can be secured on an annual registration form; and

b. Communication with a Youth via email, Facebook, or other on-line social networks will be monitored closely. Access of ministry personnel will be made available to the ministry lead as to be transparent. Every effort will be made to obtain parental permission but in cases of outreach ministry where parents are not part of SGC, ministry personnel will do their due diligence in keeping conversations that will demonstrate and model purity, integrity, transparency and accountability.

Purposes and Intent

Spring Garden Church is collecting and retaining this personal information for the purpose of enrolling your child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our organization. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Spring Garden Church to limit the information collected, or to view your child’s information, please contact us.

Parent/Guardian Consent *

I/We have read, understood and agree with the previous information above. All information given is true to the best of my/our knowledge for the Discipleship Ministry program in both Children and Youth. By checking the "yes I understand" box below, I/we consent to cover and allow my/our child to participate in the Discipleship Ministry program for the following year. In addition, this registration form will serve as a Letter of Consent to receive emails as mandated by Canada’s Anti-Spam Legislation laws, as well as the yearly permission form for all off-site activities (e.g. Laser Tag, CBOQ Raptors Game etc) for youth. However a separate Informed Letter of Consent may be sent home for certain off-site excursions (e.g. Youth Retreats).
